Adding Fields to a Form Design

A wide range of fields are supported in the platform to help you gather the information you need. You can customise your form designs by adding fields as required.

To Add a Field to a Form Design

1. Navigate to Engagement >> Form Designs

2. Click Edit on the Form Design that you would like to add a field to.

3. Click Add Field. You will then be prompted to select a field type (please see the table below for information on each field type).

What fields can I add to a Form Design?

Information Fields	Field Type	Purpose	Example
	Heading	The title of a section - Headings can be used to break-up your form sections.	'Achievements' (This will be above a question asking employees what they are proud of having done recently)
	HMTL	This is a highly versatile field which can contain images, videos, external links or formatted text	The organisation's logo, a link to a policy stored on a document control platform, instructional text formatted with company branding
	Performance Report Link	Generates a performance report on the subject. The contents and timeframe covered is determined through the form design	An employee can review a report of their performance in the last 12 months before giving themselves anoverall rating

Quantitative Fields	Radio Group	Asks the respondent to select one from a set of options to answer the question	'I have read and understood the document' (Yes, No, Help Required)
	Rating	Asks the respondent to select from a set of options, each with an incremental value linked to each option	'Please rate your direct report's teamwork against your expectations (Below, Almost, Meeting All, Above, Exceeding all)
	Yes/No	Asks the respondent to select one of two options to answer the question	'Have you been introduced to your manager yet?'
	Checkbox Group	Allows the respondent to select more than one option from a list	'What do you enjoy all working here? Select all that apply'
	Select	Gives the respondent a list of options to select from a drop-down list	'Which of our company benefits do you value most? Select from the list below'
	People Dropdown	Allows the respondent to select an employee from a list (this can include Current, or Current and Past Staff	'Who do you nominate for employee of the year award'

Qualitative Fields

Text Area

Encourages the respondent to provide a longer free-form text response

'Please give a brief update on your progress towards your goals'

Restricted Response Fields	Date	Asks the respondent to answer a question with a date (this can be limited to a certain range through the form design)	'Meeting Completed On:' (after Instructions asking a manager to have a meeting with their direct report)
	Email	Asks the respondent for an email address
	Month	Asks the respondent to choose from a drop-down list of 12 months to answer the question
	Number	Asks the respondent to answer with a number (this can be limited to a certain range through the form design)
	URL	Asks the respondent for a website address	'Please share your favourite podcast's homepage'
	Document upload	Allows the respondent to upload a document	During the on-boarding process you may provide a tax declaration form and super choice form. The employee can download these, complete them and upload them as part of their responses removing the need for these to be sent separately to an email address.

When you add a new field, you will have the option to customise a number of details. These will depend on the type of field selected. Common details include:

• Label – the question you need answered. It appears beside the field and should make it clear to the person completing the form exactly what information is required. Most fields require a label.
• Size – determines how large the label will be. It’s a good idea to maintain consistency, unless you want something to really stand out.
• Options – the list of answers the submitter may choose from. Radio Groups, Ratings and Checkboxes all require a list of options.
• Required – ticking this box makes the field a required one. The form cannot be submitted if required fields are not completed.

6. Complete the required details to customise the field (this will depend on the field type selected).

7. Select Save at the bottom of the page.

8. Your new field will appear at the bottom of the list. To move it, drag the field up or down.

9. If you require a duplicate field, simply click Duplicate on the right-hand side of the field name.

10. Preview your form at any time by clicking Preview Form at the top of the editing page.
