Forms are anything sent out via Pulses, Workflows, or Tasks. Once completed by employees, these can be found in one of two places in the User Profile.
Any completed form will always live on the profile of the person it is about.

Feedback
Forms completed by a user themselves will be stored in the Feedback tab on their profile.
Manager's Log
Forms completed about the user by anyone other than the user (supervisor or colleague) live on the Feedback Section of the Manager's Log.
What's the Difference Between the Two?
It's easy to get confused between the two sections of Feedback.
The Feedback Tab on your profile is different to the Feedback Section in the Manager's Log Tab.
Say for example a user was issued a monthly check-in form. Once they complete that form, it would be stored under their Feedback tab.
Then if a Leader's check-in form was issued to their supervisor upon completion, and it was filled out by said supervisor, then it would live under the Feedback Section of the Manager's Log on the employee profile (because it's about the employee by someone else).
The reason these are split into two sections is because by default manager's don't have access to an employees feedback tab; and vice versa, employees don't have access to the manager's log.
This is for security reasons, as there may be information sensitive or not appropriate to share with the employee or manager.
